Clinical Assessment
The American Psychological Association defines clinical assessments as the systematic evaluation and measurement of psychological, biological, and social factors in a person presenting with a possible psychological disorder, which our team is credentialed to perform.
Temporary Housing
This short-term housing solution is designed to help those experiencing acute instability, such as homelessness, domestic issues, or mental health crises, by providing a safe and secure environment. Over the course of 15 days, individuals receive not only shelter but also access to critical services, including mental health care, substance abuse support, and case management. This serves as a vital bridge, offering stability during a vulnerable time while laying the groundwork for more sustainable recovery and integration into the community
Connection To Additional Resources
Connection to additional resources within a Community Stabilization program is an essential component that empowers individuals to achieve long-term stability beyond immediate support. This aspect of the program focuses on linking participants with a wide range of services tailored to their specific needs, such as mental health care, substance abuse treatment, employment assistance, legal aid, and housing solutions. By connecting individuals with these resources, the program ensures they have access to ongoing support and opportunities that promote self-sufficiency and well-being. Case managers work closely with participants to identify the most appropriate services, helping them navigate complex systems and apply for benefits or programs they may be eligible for. This holistic approach addresses not only the immediate crisis but also the underlying issues that contribute to instability
Life/Independent Living Skills
Equips individuals with the practical tools they need to regain control of their daily lives and maintain long-term stability. These skills are essential for individuals transitioning out of crisis situations, such as homelessness, addiction, or mental health challenges, and aim to foster independence and self-sufficiency. The program offers personalized support in areas like financial management, meal planning, personal hygiene, job readiness, and time management. By teaching these critical life skills, participants build the confidence and capabilities to manage their households, secure employment, and navigate community resources on their own. Case managers and support staff work closely with individuals to ensure that these skills are adapted to their unique situations, providing ongoing guidance and encouragement. The goal is to empower participants to take charge of their lives, create stability, and ultimately thrive in their communities, reducing the likelihood of future crises.
Medication Managed
Medication management is a level of outpatient treatment that involves the initial evaluation of the patient’s need for psychotropic medications, the provision of a prescription, and ongoing medical monitoring related to the patient’s use of the psychotropic medication by a qualified physician/prescriber.
Psychiatric Evaluations
A psychiatric evaluation may be needed to diagnose emotional, behavioral, or developmental disorders. Questions from the doctor are based on what you tell them, what they see, and what they read in your medical chart. Filling out questionnaires or other forms about your family history, physical and mental health history, and lifestyle may also be used for a full comprehensive evaluation.
What Is Community Stabilization Like?
Community Stabilization services are short-term and designed to support an individual and their natural support system following contact with an initial crisis response service or as a diversion to a higher level of care. Providers deliver community stabilization services in an individual’s natural environment and provide referral and linkage to other community based services at the appropriate level of care.
Interventions may include brief therapeutic and skill building interventions, engagement of natural supports, interventions to integrate natural supports in the de-escalation and stabilization of the crisis, and coordination of follow-up services. Coordination of specialized services to address the needs of co-occurring intellectual/developmental disabilities and substance use are also available through this service. Services should involve advocacy and networking to provide linkages and referrals to appropriate community-based services and assisting the individual and their family or caregiver in accessing other benefits or assistance programs for which they may be eligible.
The goal of Community Stabilization services is to stabilize the individual within their community and support the individual and/or support system during the periods 1) between an initial Mobile Crisis Response and entry in to an established follow-up service at the appropriate level of care 2) as a transitional step-down from a higher level of care if the next level of care service is identified but not immediately available for access or 3) as a diversion to a higher level of care
